Dr. Heather has a strong passion for providing the best possible dental care to her patients. While a large part of that focuses on delivering excellent clinical care, Dr. Heather firmly believes that ultimately the best care emerges from getting to know the individual patient, their desires and needs. She endeavors to provide gentle care with both short-term and long-term goals in mind.
Our staff truly believes in preventative and conservative treatment methods which includes education and providing patients with the tools needed for overall oral health. Dr. Heather and her team have a strong passion for professional development through education and strive to stay up-to-date on the latest dental technology and treatment.
Dr. Heather grew up in small town Eyota, MN, located about 20 minutes to the east of Rochester, MN. She completed her undergraduate studies at the University of Wisconsin – Eau Claire. Then attended dental school at the University of Minnesota where she met her husband, Tyler Kalmuck. After graduation, they moved to Iowa City, IA where she practiced for 4 years. The couple married in 2019 and in July 2020, Dr. Heather and her husband, an Oral Surgeon at Sheboygan Oral & Maxillofacial Associates moved to Sheboygan. They are eager to put down roots and become a part of this community. Dr. Heather likes to spend free time outdoors whenever possible. Hiking, boating, biking, and snowshoeing are some of her go to activities. To relax, she enjoys watching movies, playing games and doing puzzles. She loves traveling and trying new foods and restaurants with her husband, Tyler.
Dr. Heather is a member of the American Dental Association, the Wisconsin Dental Association, Sheboygan County Dental Association and the Academy of General Dentistry.
Douglas R. Martin, D.D.S.

Dr. Martin is a graduate of Marquette Dental School in Milwaukee, Wisconsin. He enjoys helping you find solutions to your dental needs in the most conservative pain-free way. Your overall dental health is of utmost concern to Dr. Martin and his staff, and they appreciate the trust you place in them.
While being a gentle and caring practitioner with exceptional clinical skills, Dr. Martin integrates technology within all phases of your visit to create a truly pleasant experience.
Dr. Martin and his team are true believers that preventative care and education are the keys to optimal dental health. That’s why they focus on thorough exams – checking the overall health of your teeth and gums, performing oral cancer exams, and taking x-rays when necessary. Dr. Martin also knows that routine cleanings, flossing, sealants, and fluoride are all helpful in preventing dental disease. Not only is he focused on the beauty of your smile, he is also concerned about your health. A review of your medical history helps him stay informed of your overall health, any new medications, and any illnesses that may impact your dental health.

Dr. Martin is a member of the American Dental Association, the Sheboygan County Dental Society, and the Wisconsin Dental Association.
Cris received her B.S. in Dental Hygiene from the University of Iowa. She’s married and has three adult children, and she’s resided in Sheboygan since 1981. Cris says that during the 26 years she’s been employed in this practice, she’s been blessed to be able to provide dental care for multiple generations!
Denise is originally from Upper Michigan and received her degree in Dental Hygiene from Ferris State University in Big Rapids, MI. For the past 20 years, she’s lived in Sheboygan where she’s raised, two daughters. Denise loves her career as a dental hygienist. In fact, she still has the “What I Want To Be When I Grow Up” poster she made in fifth grade. She enjoys working with her patients, helping them achieve optimal dental health. She says, “I’m pleased Dr. Martin sets such high standards for our practice. My goal as part of Dr. Martin’s dental team is to provide the very best dental care to each of our patients.”
Chris has lived in Wisconsin all her life, in various cities. She received her degree as a Dental Assistant in 1980 from Lakeshore Technical Institute (now Lakeshore Technical College). Chris describes herself as a “roving, all-purpose assistant, not only working with Dr. Martin at the chairside but helping out the hygienists and Annie as needed around the office.”
Front Office Manager
Annie is a native of Wisconsin and has called Sheboygan home for the more than 15 years. She and her husband have five children and love to spend time together in the outdoors. Annie enjoys cooking, baking, reading, and watching movies in her free time. It was her love of working with people that inspired Annie to attend Lakeshore Technical College to become a Dental Assistant. She joined Dr. Martin’s team in May of 2013 as a Dental Assistant. In October 2013, Annie took over the Front Office Manager position. Her goal is to help each patient in any way she can.
